Revision rounds are the leak. They are the most common and least noticed way an agency gives work away, so they are counted per deliverable against the number that was agreed, every day.
Signed before the work, or it is a collection problem. A change order agreed before anyone starts is a normal commercial conversation. The same change raised after the work is done is an argument the firm usually loses, because the only leverage it had was that the work had not happened yet.
A failed assumption is a change. That is the entire reason assumptions get written into a statement of work. If the client's content arrives six weeks late, or the approver turns out to be three approvers, the fee rested on something that is no longer true.
The schedule impact is never omitted. Extra work moves other dates. A change order that hides that has promised the client two incompatible things.
One false change order costs every real one. Anything genuinely inside scope is left alone, quietly. Credibility here is the working capital of the seat.
The pattern matters more than the case. The same assumption failing on five engagements is a template problem, and fixing the template is worth more than all five change orders.
