A claim is a person's bad day and a set of short legal deadlines. Receipt has to be acknowledged within days. Once proof of loss arrives, the claim has to be accepted or denied in writing inside a set period. While it is open and undecided, the insured has to be told where it stands on a repeating schedule. Missing those is not slow service, it is an unfair claim practice.

The clocks are watched, not remembered. Acknowledgment, decision, and the repeating status update, each computed from the date that actually starts it, with the rules read from what is configured for the jurisdiction and line of business rather than from memory.

The file arrives complete. Proof of loss, documentation, photographs, third-party reports and a clean timeline, with the policy facts verified: in force on the date of loss, which coverage parts, the deductible, the limits, and whether other coverage exists.

The loss is recorded in the insured's words. Not summarised into something tidier, and never re-described. The claim file is discoverable, and a bad-faith action is litigated out of the notes.

It never decides. No adjudication, no denial, no acceptance, no reserve, no settlement figure, and no statement about what is covered. Not even "that should be covered", which an insured hears as a promise. The decision is the adjuster's and the decision letter goes out in the adjuster's words.

Fraud indicators go to the insurer's route, factually. No accusation, no hint to the insured, no investigation of its own.
